De La Salle University puts premium value on the privacy and security of personal data entrusted by its students for legitimate purposes. De La Salle University recognizes their responsibilities under the Republic Act No. 10173, also known as the Data Privacy Act of 2012, with respect to the data they collect, record, organize, update, use, consolidate from the students. The personal data was obtained from the students. The personal data obtained from this portal/form is entered and stored within the university’s authorized information and communications system and will only be accessed by the authorized personnel. The University has instituted appropriate organizational security measures to ensure the protection of the students’ personal data. As such, I agree and authorize Computer Studies Government (CSG) to: 1. Use my personal data for any organizational activity authorized by the UNIVERSITY and CSG for the Academic Year 2025-2026. 2. Retain my information through my entire stay in the organization or at such a time that I submit to CSG a written cancellation whichever is earlier. I agree and understand that my information will be deleted or disposed of at the end of the Academic Year 2025-2026. Note: In case the data is required by law (and/or approved by DLS) to have a longer retention period, please specify the date until which the date shall be retained. 3. Store a copy of my personal data in the CSG Google Drive. 4. Inform me of where my information will be used through a written report of my data, upon the submission of a written request addressed to CSG. 5. I understand and am assured that necessary precautions will be taken by the UNIVERSITY and CSG to protect my personal information. By filling out this form, I hereby give my consent to CSG to use my data for the purposes of their Student Services initiatives.
